随着国家基本公共卫生服务的正式实施，慢性病患者健康管理就是其中一项重要项目。国家明确要求社区卫生服务机构为辖区内确诊的高血压或2型糖尿病患者每年免费提供4次面对面随访服务和1次较全面的健康检查服务。我院为规范慢病随访，引进公卫随访包。通过手机就能进行随访，无需纸质版。大大节省了乡医随访时间，并且真实性有了很大的提升。

　　With the formal implementation of basic public health services in the country, the health management of chronic disease patients is one of the important projects.
